The primary purpose of the Clinical Care Coordinator position is to serve as the coordinator and practitioner of the clinic’s healthcare team(s) within the Department, utilizing specialized knowledge, skills, and competencies in behavioral sciences, education counseling, and/or patient management. Develops patient programs and course curricula based on needs assessments; plans and coordinates the delivery of comprehensive services to patients in collaboration with all professional components of the clinic and in accordance with accepted standards of care and department protocols.
Minimum of Bachelor’s Degree in Nursing (BSN) or a related health care field. A Master’s Degree in Nursing, Health Care Administration, Public Health, or a related field is preferred. A minimum of three years of full-time experience in the field of specialization and experience in a hospital-based practice are required. Effective team leadership and interpersonal skills are required. National Certification in Medical/Surgical Nursing or another Nursing Specialty highly preferred. Must hold CPR certification.
