Under the direction of Executive Director Community and Population Health Services, the Manager Community Healthcare is responsible for supervising a Community Healthcare team, including but not limited to, Healthcare Chaplains and Patient Navigators, as part of University Hospital’s Familiar Faces program. University Hospital’s Familiar Faces program offers an innovative, evidence-based health care program that seeks to improve health outcomes for low-income, high-risk patients, by providing structured advocacy, social support, and navigation through the healthcare system in order to help them reach their health goals.
Bachelor’s degree in Public Health, Administration, Communication or related field required. Four (4) years of progressively more responsible administrative experience required, preferably including work with a non-profit or community-based organization. Experience working collaboratively and cooperatively with clinical social work or case management preferred. Understanding of the healthcare and social services systems including processes for enrolling in public benefits a plus. Knowledge of the resources and community-based organizations in the Greater Newark community required. Experience analyzing reports and using data to drive performance results. Must possess excellent written and verbal communication skills and experience with data management and/or metrics. Proficiency with Microsoft Office Suite required. Additional related experience may be substituted for the degree on a year for year basis.

PHYSICAL DEMANDS: This position requires sitting, talking and using of hands > 2/3 of the time; standing, walking, stooping, and crouching <1/3 of the time. Position also requires lifting up to 25-50 lbs. <1/3 of the time. The special vision requirements are as follows: close vision, distance vision, color vision, peripheral vision, depth perception, and ability to adjust focus.
WORK ENVIRONMENT: This position is exposed to the following environmental conditions: work near moving mechanical parts, fumes or airborne particles, risk of electrical shock, and vibration under 1/3 of the time. Moderate noise is typical for this work environment.
