The primary purpose of the Supervising Advanced Practice Nurse for Palliative Care is to provide a collaborative and interdisciplinary environment, contributes professional and clinical nursing expertise to nursing units and/or patient
population to ensure the delivery of high quality nursing care. Scope of practice will encompass:

‐ Education/Coach to patients, families and other health care professionals

The primary purpose of the Case Management Coordinator position is to coordinate the patient’s progress through the system. He/she collaborates with physicians and clinical staff to ensure resource utilization is appropriate to the patient’s clinical and psychosocial needs and that it remains within covered benefits.
